Several achievement bugs were fixed in the August 21 patch (and the "ten Nyanchi Crunchi boxes" grind was made humane on August 22). Make sure the game is updated, keep a separate good save, and retest. If a counted requirement still will not pop, check the fixes page.
What is the rarest achievement?
The long-haul ones: Many Hoppy Returns! (1,000 beers), Annihilation (999 garbage bags) and The Collector (all foil cards). Everything else falls out of a normal completion run.
Can I get all achievements in one playthrough?
Yes — nothing is missable. All 40 are cumulative counters or collection goals in a single ongoing save.
